Beyond the Labs: Why "Normal" Blood Sugar Can Still Mean Insulin Resistance

Most people believe that if their blood sugar is normal, they're in the clear.
It's one of the biggest misconceptions in healthcare.
The reality is that your blood sugar can remain completely normal for years while your body is quietly becoming insulin resistant.
By the time blood sugar begins to rise, metabolic dysfunction has often been developing for a decade or longer.

Blood Sugar Is Only Part of the Story
When your healthcare provider orders routine lab work, you may see:
Fasting glucose
Hemoglobin A1c (HbA1c)
These are valuable tests, but they're late markers.
Think of it like checking the water level in a sink.
Blood sugar tells us how much water is in the sink.
But it doesn't tell us how hard the faucet has to work to keep the water level there.
That faucet is insulin.
What Is Insulin?
Insulin is a hormone made by your pancreas.
Its job is to move sugar from your bloodstream into your cells, where it's used for energy.
In a healthy body, only a small amount of insulin is needed to keep blood sugar in a normal range.
But over time, our cells can become less responsive to insulin.
This is called insulin resistance.
When that happens, your pancreas works harder and harder, producing more insulin just to keep your blood sugar looking "normal."
Your glucose level may look great—but your insulin level may already be working overtime.
Why Does Insulin Resistance Matter?
Insulin resistance is one of the earliest drivers of chronic disease.
It has been linked to:
Weight gain, especially around the abdomen
Difficulty losing weight
Prediabetes and type 2 diabetes
High triglycerides
Low HDL cholesterol
Fatty liver disease
High blood pressure
Heart disease
Polyendocrine metabolic ovarian syndrome (PMOS)
Cognitive decline
Chronic inflammation
Many people develop insulin resistance years before they ever receive a diagnosis of diabetes.
The Tests That Can Detect It Earlier
Fasting Insulin
This is one of my favorite preventive tests.
It tells us how much insulin your body is producing after an overnight fast.
A fasting glucose of 90 mg/dL may appear perfectly normal.
But if your insulin is elevated, it tells us your body is working much harder than it should to keep that glucose normal.
Ideally, we want insulin to be low and efficient, not high and compensating.
Intact Insulin
This test offers an even more detailed look at insulin production and can help identify metabolic dysfunction earlier than glucose testing alone.
It's particularly useful when we suspect insulin resistance despite normal glucose or A1c levels.
Hemoglobin A1c (HbA1c)
HbA1c reflects your average blood sugar over the past two to three months.
While it's excellent for diagnosing diabetes, it may remain within the normal range during the early stages of insulin resistance.
In other words, HbA1c tells us how blood sugar has behaved—but not how much insulin it took to get there.
Why You Can Feel Unwell Even With "Normal" Labs
Many patients tell me:
"I'm exhausted after meals."
"I crave sugar all day."
"I can't lose weight no matter what I do."
"I have brain fog."
"I'm hungry again an hour after eating."
These symptoms can be signs of insulin resistance—even when fasting glucose and HbA1c are reported as normal.
That's why symptoms always matter.
Labs should support the clinical picture, not replace it.

What Causes Insulin Resistance?
Several factors can contribute, including:
Diets high in ultra-processed foods and added sugars
Physical inactivity
Poor sleep
Chronic stress
Excess body fat, particularly around the waist
Genetics
Hormonal changes, especially during menopause
Chronic inflammation
The good news?
Insulin resistance is often reversible.
The Good News: You Can Improve It
One of the most encouraging things about insulin resistance is that lifestyle changes can make a tremendous difference.
For many people, improving insulin sensitivity includes:
Eating enough protein at each meal
Prioritizing fiber-rich vegetables and whole foods
Strength training to build muscle
Walking after meals
Improving sleep quality
Managing chronic stress
Maintaining a healthy body composition
When appropriate, certain supplements and medications may also play a role, but lifestyle remains the foundation.
The Bottom Line
Blood sugar is only one piece of the puzzle.
By the time glucose begins to rise, insulin may have been elevated for years.
Looking at fasting insulin, intact insulin, and the whole metabolic picture allows us to identify problems earlier—when they're often easier to reverse.
