Beyond the Labs: Inflammation — The Silent Driver of Chronic Disease

Why hs-CRP May Be One of the Most Important Labs


Inflammation gets blamed for almost everything these days.

Fatigue. Joint pain. Heart disease. Diabetes. Autoimmune conditions. Brain fog. Accelerated aging.

But what exactly is inflammation—and how do you know if you have too much of it?

Sometimes inflammation is obvious. If you sprain your ankle and it becomes red, swollen, warm, and painful, you're seeing your immune system at work.

But there is another type of inflammation that can be much harder to recognize.

It's called chronic, low-grade inflammation, and it can exist quietly for years without obvious symptoms.

One simple blood test can help us detect it:

High-Sensitivity C-Reactive Protein, or hs-CRP.


First, Inflammation Isn't Always Bad


Inflammation is actually an essential part of your body's defense system.

When you have an infection or injury, your immune system activates an inflammatory response to help protect and repair your body.

That's exactly what it's supposed to do.

The problem occurs when that response doesn't fully turn off.

Instead of inflammation being a temporary response to an injury or infection, the immune system can remain persistently activated at a lower level.

Think of it like a fire alarm.

When there's a fire, you want the alarm to sound.

But you don't want it going off 24 hours a day.


What Is hs-CRP?


CRP stands for C-reactive protein, a protein produced primarily by the liver in response to inflammatory signals in the body.

The high-sensitivity CRP (hs-CRP) test can detect much smaller elevations in CRP than a standard CRP test.

That makes it particularly useful when we're looking for low-grade inflammation and evaluating cardiovascular risk.

In simple terms:

hs-CRP gives us a window into how much inflammatory activity may be occurring in the body.

But there's an important distinction.

hs-CRP doesn't tell us why inflammation is present.

It tells us that we may need to look deeper.


What Does hs-CRP Have to Do With Heart Disease?


For decades, cardiovascular risk assessment focused heavily on cholesterol.

We now understand that atherosclerosis—the process that leads to plaque buildup inside arteries—is not simply a cholesterol-storage problem.

Inflammation plays an important role in the development and progression of atherosclerosis.

That's why two people with similar cholesterol levels can have very different cardiovascular risks.

One may have very little underlying inflammation.

The other may have evidence of metabolic dysfunction, vascular inflammation, smoking exposure, obesity, or other factors contributing to atherosclerosis.

hs-CRP gives us another piece of that cardiovascular puzzle.

It should not replace cholesterol testing, ApoB, Lp(a), blood pressure assessment, glucose evaluation, or other risk factors.

It adds context.


What Can Raise hs-CRP?


This is where interpretation matters.

An elevated hs-CRP does not automatically mean you have heart disease.

Many things can temporarily or chronically increase CRP, including:

  • A recent infection

  • Injury or surgery

  • Autoimmune or inflammatory conditions

  • Smoking

  • Obesity, particularly visceral fat

  • Poorly controlled metabolic disease

  • Periodontal disease

  • Chronic inflammatory conditions

  • Intense exercise shortly before testing

This is why I don't interpret hs-CRP in isolation.

If someone has an unexpectedly high result while recovering from the flu, for example, that value may tell us very little about their usual cardiovascular inflammation.

Sometimes the appropriate next step is simply to repeat the test when the person is well.


The Connection Between Insulin Resistance and Inflammation

This is where our previous Beyond the Labs topics begin to connect.

Insulin resistance and chronic inflammation often travel together.

Visceral fat—the fat stored around internal organs—is metabolically active tissue. It can release inflammatory signals that contribute to systemic inflammation and worsening insulin resistance.

And the relationship can become a cycle:

Insulin resistance → metabolic dysfunction → inflammation → worsening insulin sensitivity.

This is one reason I like looking at markers together rather than individually.

A fasting glucose may look normal.

But if fasting insulin is elevated, triglycerides are rising, HDL is falling, waist circumference is increasing, and hs-CRP is elevated, we're seeing a very different metabolic picture.


What About "Optimal" hs-CRP?


You may see cardiovascular risk categories that generally describe hs-CRP levels below 1 mg/L as lower risk, 1–3 mg/L as intermediate risk, and above 3 mg/L as higher risk.

But there's an important caveat:

A lab value should never be interpreted without context.

A significantly elevated hs-CRP may reflect an acute infection or another inflammatory condition rather than someone's baseline cardiovascular risk.

Instead of chasing a perfect number, I want to understand the pattern.

Is it persistently elevated?

What does the rest of the metabolic picture look like?

Are there cardiovascular risk factors?

Are there symptoms or another explanation for inflammation?

That's much more useful than labeling one result as simply "good" or "bad."


Can You Feel Chronic Inflammation?


Sometimes.

But not always.

Some people with chronic inflammation may experience:

  • Persistent fatigue

  • Brain fog

  • Joint or muscle discomfort

  • Poor recovery

  • Digestive symptoms

  • Difficulty losing weight

  • Changes in metabolic health

But these symptoms are nonspecific and can occur for many different reasons.

And some people with elevated inflammatory markers feel perfectly well.

That's exactly why preventive testing can be valuable.

How you feel today doesn't always tell us what is happening metabolically beneath the surface.


How Do We Lower Chronic Inflammation?


The answer usually isn't one supplement.

The biggest improvements often come from addressing the fundamentals:

  • Improving insulin sensitivity

  • Regular resistance and aerobic exercise

  • Maintaining a healthy amount of visceral fat

  • Eating a minimally processed, nutrient-dense diet

  • Getting adequate fiber

  • Avoiding smoking

  • Prioritizing consistent, restorative sleep

  • Managing blood pressure and metabolic health

  • Treating underlying medical or inflammatory conditions when present

Depending on the individual, we may also need to investigate other contributors.

The goal isn't to suppress every inflammatory signal.

The goal is to understand why inflammation is elevated in the first place.


Looking at the Bigger Picture


At Zivi Health, hs-CRP is never viewed as a standalone answer.

Depending on the patient, cardiovascular and metabolic evaluation may also include:

  • Standard lipid panel

  • ApoB

  • Lipoprotein(a) [Lp(a)]

  • LDL particle number and size

  • Fasting glucose

  • Fasting insulin

  • Hemoglobin A1c

  • LP-PLA₂ activity

  • Myeloperoxidase (MPO)

  • Oxidized LDL

  • Blood pressure

  • Waist circumference and body composition

  • And other individualized markers

And sometimes one of the most useful cardiovascular tests isn't a blood test at all—such as a coronary artery calcium (CAC) scan or coronary computed tomography angiography (CCTA) in appropriately selected patients.

The question isn't:

"Is this one lab normal?"

The better question is:

"What does the entire picture tell us about this person's health and future risk?"


The Bottom Line


Inflammation isn't the enemy.

It's a normal and necessary part of being human.

But persistent, low-grade inflammation can be an important clue that something beneath the surface deserves attention.

hs-CRP is a simple blood test that can help us identify that signal—particularly when evaluating cardiovascular and metabolic health.

It doesn't diagnose the cause.

It doesn't predict your future by itself.

And it shouldn't be interpreted in isolation.

But when combined with your history, symptoms, metabolic health, cholesterol markers, lifestyle, and other risk factors, it can provide another valuable piece of the puzzle.
