Beyond the Labs: Understanding Thyroid Labs Beyond TSH

If you've ever been told:

"Your thyroid is normal."

...yet you still struggle with fatigue, weight gain, brain fog, hair loss, constipation, or feeling cold all the time, you're not alone.

One of the most common reasons patients come to Zivi Health is because they continue to experience symptoms despite being told their thyroid labs are "normal."

Often, only one test was checked: TSH.

While TSH is an important part of the picture, it doesn't tell the whole story.

What Is TSH?

TSH stands for Thyroid Stimulating Hormone.

Here's the surprising part:

TSH isn't actually a thyroid hormone.

It's a hormone produced by the pituitary gland in your brain. Its job is to signal your thyroid gland to produce thyroid hormones.

Think of it like a thermostat in your home.

The thermostat tells your furnace when to turn on, but it doesn't tell you whether warm air is actually reaching every room.

In the same way, TSH tells us what message the brain is sending—but not necessarily how well your body is responding.

What Does the Thyroid Actually Produce?

Your thyroid gland primarily makes two hormones:

  • T4 (Thyroxine) – the storage hormone

  • T3 (Triiodothyronine) – the active hormone

Think of T4 as money sitting in your savings account.

It's valuable—but you can't spend it until it's transferred into your checking account.

That's where T3 comes in.

T3 is the hormone that actually enters your cells and helps regulate:

  • Energy production

  • Metabolism

  • Body temperature

  • Heart function

  • Digestion

  • Mood

  • Memory and concentration

  • Hair, skin, and nails


The Missing Piece: Conversion

Most of the hormone produced by your thyroid is T4.

Your body must convert T4 into T3 before it can be used.

This conversion happens primarily in the liver, kidneys, muscles, and other tissues.

Sometimes the thyroid gland is making enough hormone—but the body isn't converting it efficiently.

This can leave someone with persistent symptoms despite a "normal" TSH.

Why Conversion Can Slow Down

Several factors can reduce T4-to-T3 conversion, including:

  • Chronic stress

  • Poor sleep

  • Inflammation

  • Nutrient deficiencies (such as selenium, zinc, and iron)

  • Liver dysfunction

  • Chronic illness

  • Certain medications

  • Calorie restriction or prolonged dieting

Addressing these underlying factors is often just as important as looking at the thyroid itself.

Reverse T3: The "Brake Pedal"

Sometimes, instead of converting T4 into active T3, the body converts it into Reverse T3 (rT3).

Reverse T3 is inactive.

Think of it as putting the brakes on your metabolism.

This can happen during periods of:

  • Chronic illness

  • Significant stress

  • Poor nutrition

  • Severe calorie restriction

  • Overtraining

  • Inflammation

Reverse T3 is part of the body's natural survival response, but if the underlying issue persists, it may contribute to symptoms of low thyroid function.

Thyroid Antibodies: Looking for Autoimmunity

Another important piece that is often overlooked is thyroid antibodies.

The two most commonly measured are:

  • Thyroid Peroxidase Antibodies (TPOAb)

  • Thyroglobulin Antibodies (TgAb)

These tests help determine whether the immune system is attacking the thyroid gland.

Hashimoto's thyroiditis is the most common cause of hypothyroidism in the United States.

In many people, thyroid antibodies become elevated years before TSH becomes abnormal.

Identifying autoimmune thyroid disease early provides an opportunity to monitor thyroid function closely and address lifestyle factors that may support overall health.

Looking Beyond the Numbers

Laboratory values are important—but they don't replace the whole person sitting in front of us.

Symptoms still matter.

When evaluating thyroid health, I consider:

  • Your symptoms

  • Your medical history

  • Your physical examination

  • Family history

  • Lifestyle factors

  • Nutritional status

  • Inflammation

  • And, when appropriate, a more comprehensive thyroid panel

Every piece helps us better understand what's happening beneath the surface.

A More Complete Thyroid Evaluation

Depending on your history and symptoms, my recommendation for initial thyroid evaluation:

  • TSH

  • Free T4

  • Free T3

  • Reverse T3

  • Thyroid Peroxidase Antibodies (TPOAb)

  • Thyroglobulin Antibodies (TgAb)

Each test provides a different piece of the puzzle.

No single number tells the entire story.

The Bottom Line

TSH is an excellent screening tool—but it isn't a complete evaluation of thyroid health.

Understanding thyroid function means looking beyond one lab value and considering hormone production, hormone conversion, autoimmune activity, and, most importantly, how you're actually feeling.
